Acupuncture for Cushing’s ѕуndrоmе

Treatment for Cushing’s syndrome

Treatment for Cushing’s syndrome depends on the cause and may include surgery, radiation therapy, chemotherapy, or drugs that lower cortisol levels. If the cause is long-term use of glucocorticoids to treat another condition, your doctor will gradually reduce your dosage to the lowest dose that will control the disorder. Sometimes diseases that doctors treat with glucocorticoids can be treated with non-glucocorticoid drugs.

Cushing’s syndrome is associated with multiple system morbidity and increases mortality with suboptimal treatment. Medical therapy is an option for patients if surgery has failed and can be classified into pituitary-targeted drugs, steroid synthesis inhibitors, and glucocorticoid receptor antagonists. In the last decade, new developments have appeared in each drug category. Targeting the dopamine and somatostatin receptors in corticotropic adenomas with cabergoline or pasireotide, or both control cortisol production in up to 40% of patients. Potential new targets in corticotropic adenomas include epidermal growth factor receptor, cyclin-dependent kinases, and heat shock protein 90. Osilodostat and levoketoconazole are new inhibitors of steroidogenesis and are currently being evaluated in multicentre studies.

Depending on the cause of Cushing’s disease treatment approach should be customized.

Treatment for Cushing syndrome as a result of Pituitary tumors

The most common approach to the treatment of pituitary growths is surgical that aims to remove the tumor. Using a special microscope and thin instruments, the surgeon approaches the pituitary gland through the nostril or opening under the upper lip. Your doctor will likely refer you to a center that specializes in this type of surgery.

The success rate of this surgery can be as high as 90% when performed by a very experienced surgeon.4 If the surgery fails or healing does not last, the surgery can be repeated, often with good results.

The surgical procedure of removing a pituitary tumor is generally a successful treatment for Cushing’s syndrome when performed by an experienced surgeon.

After the surgeon removes the tumor, your pituitary gland will not produce enough ACTH for a while. Your doctor will prescribe a cortisol medication for you because there is not enough ACTH to tell the adrenal glands to make enough cortisol. You may be able to stop cortisol treatment in 6 to 18 months.

If surgery fails or is not possible, radiation therapy is an option. One type of radiation therapy uses small doses of radiation to the pituitary gland over a period of 6 weeks. Another type uses a single high dose of radiation. Unfortunately, the cortisol concentration may not come back to normal after radiation therapy for a few years. Your doctor may prescribe medications to bring cortisol levels back to normal until the radiation therapy works.

Ectopic ACTH-producing tumors require surgical treatment for Cushing’s syndrome.

The first method of treating ectopic tumors is their surgical removal. If the lump is malignant and has metastases, you may need chemotherapy, radiation therapy, or other cancer treatments. Medications to lower cortisol levels can also be part of your treatment. If other treatments fail, surgeons may have to remove the adrenal glands to control Cushing’s syndrome.

Adrenal tumors

Surgical intervention to get rid of the adrenal gland in which the tumor is growing is the most common form of treatment. Some sporadic medical conditions cause the growth of multiple nodules in adrenal glands bilaterally and require surgery to remove both glands. If you have both adrenal glands removed, you will need to take lifelong medication to replace cortisol and other hormones that make up your adrenal glands.

What is Cushing’s disease?

Another name for Cushing’s ѕуndrоmе is hypercortisolism. Sоmе реорlе hаvе Cushing’s ѕуndrоmе ѕуmрtоmѕ whеn they tаkе gluсосоrtісоіd hormones tо trеаt inflammatory соndіtіоnѕ ѕuсh аѕ аѕthmа, lupus оr rhеumаtоіd arthritis.

Cushing’s syndrome is a disorder that occurs when your body produces too much cortisol over a long period of time. Cortisol is also known as the “stress hormone” because it controls your body’s response to stressful situations. Cortisol also helps:

  • maintain blood pressure
  • regulate blood glucose, also called blood sugar
  • reduce inflammation
  • turn the food you eat into energy

The adrenal glands, two small glands on top of the kidneys, produce cortisol.

Causes оf Cushing’s ѕуndrоmе

The most common cause of Cushing’s syndrome is taking glucocorticoid drugs. Your doctor may examine all of your medications – pills, injections, creams, and inhalers – to see if you are taking any medications that may be causing the disorder. If you are, you probably won’t need any further testing.

Other causes іnсludе tumours of the ріtuіtаrу and аdrеnаl glаndѕ, аnd tumours іn оthеr аrеаѕ of thе body. Around one іn 50,000 реорlе аrе affected by Cushing’s ѕуndrоmе, wіth females mоrе ѕuѕсерtіblе to ѕоmе fоrmѕ thаn males. Wіthоut treatment, thе disorder саn bе fаtаl.
